This Privacy Policy describes how information is collected and used when you visit care.neuropathyreliefproject.com, an informational website for Panda Physical Medicine LLC, operated on the practice's behalf by Magratech Inc, dba Propelient ("we," "us").

Information we collect

• Information you provide: if you use the contact form, we collect your name, phone number, email address, and the general topic you're interested in — nothing more. The form deliberately does not ask about, and you should not submit, symptoms, conditions, medical history, or any other health information. Health questions belong in a conversation with the practice directly, through the secure channels they provide for patients.
• Usage information: standard technical data such as IP address, browser type, and pages visited, used for security (for example, rate limiting and abuse prevention) and to operate the site. This site also uses Google Analytics to understand how visitors find and use it; see "Analytics and advertising" below.

This website does not collect, store, or process patient health records or personal health information, and it is not a patient portal.
